New Patients

Your first visit is all about you — your comfort, your happiness, and your health

During your first visit, we will:

  • Review your dental and medical history forms
  • Take X-rays of your teeth and mouth
  • Conduct a doctor-performed exam
  • Clean and polish your teeth, checking your teeth and gums for any cavities or early signs of tooth decay or gingivitis

After your checkup and teeth cleaning are complete, the doctor will meet with you to discuss your X-rays, help you create your own individual treatment plan, and answer any remaining questions.

Remember to Bring These to Your First Appointment

If you are a new patient, please download, print and complete the electronic forms linked below and bring them to your first appointment. Having the forms completed before you come in helps us ensure that you are seen in a timely manner.

We understand that you have a busy schedule, so when you’re preparing to visit the dentist, please remember that your appointments can last between 45 and 90 minutes. Please arrive twenty (20) minutes prior to your appointment so that your Dental Team can complete the intake process efficiently.

Financial Information

The cost of your treatment will vary depending on your individual needs and treatment plan. We will discuss with you the cost of your treatment and each of your available payment options before you begin so that you can make the best choice for you.

Affordable Payment Plans

Your smile is yours forever, and we want to make it as easy as possible for you to receive the best dental care when you need it, so we accept most major credit cards and insurance plans. If you have questions about payments, our office staff is always available to help you with insurance claims and paperwork. We’re happy to work with your insurance provider to make sure that your coverage meets your needs and your budget.
